Saving Face After Using GLP-1s for Weight Loss

A surgical and cosmetic dermatologist breaks down how to keep facial volume, elasticity and that ‘youthful’ look.

GLP-1 receptor agonists have transformed how we treat obesity and type 2 diabetes. For many, these drugs deliver life-changing results like steady weight loss, better blood sugar control and improved self-confidence. But along with these benefits, some patients are noticing changes they didn’t expect, particularly when they look in the mirror.

“While the patients I see who have lost 20 or 30 pounds are thrilled with their results and feel healthier than ever, they also tell me their face looks thinner and more tired,” says Dr. May Elgash, a surgical and cosmetic dermatologist at Jefferson Health. “Some say they look like they’ve aged a decade overnight.”

What Is ‘GLP-1 Face’?

“GLP-1 face” isn’t a medical diagnosis but a descriptive term for the loss of facial volume, skin elasticity and youthfulness that can occur after rapid weight loss. When the fat pads that normally support the cheeks, temples and jawline shrink quickly, the skin can appear hollowed or deflated. The appearance can be stigmatizing for patients.

“Some of my patients joke that they feel like they’re wearing a sign announcing they’re on weight-loss medications because the facial changes can be so distinctive,” Dr. Elgash says.

Recent imaging studies indicate that people taking GLP-1 medications lose about 7% of midfacial volume for every 10 kilograms (22 pounds) of total weight loss. This shows that even healthy weight loss can visibly alter the face. Emerging data also suggest GLP-1 medications can affect the function of cells that maintain collagen and skin structure, potentially accelerating the visible signs of facial aging.

Together, these factors can lead to thinning, sagging and texture changes that go beyond what would be expected from weight loss alone.

Protecting Skin During Weight Loss

Dr. Elgash emphasizes that sagging effects aren’t inevitable and taking preventive steps early can make a significant difference. Here are some of her suggestions:

  • Lose weight gradually. A slower, steady pace allows the skin time to adapt.

  • Maintain adequate protein intake. Protein supports collagen and tissue strength.

  • Stay well-hydrated. Proper hydration enhances elasticity and resilience.

  • Protect from UV damage. Daily sunscreen use helps prevent additional collagen breakdown.

A targeted skin care regimen can also support the skin’s integrity during weight loss. Ingredients such as retinoids (to boost collagen turnover), vitamin C (to reduce oxidative stress and support collagen synthesis), peptides (to enhance repair) and hyaluronic acid (to maintain hydration) work synergistically to keep the skin healthy.

Treatment Options

For patients already noticing facial hollowing or skin laxity, a variety of minimally invasive options are available to help combat facial changes. These include:

  • Dermal fillers (hyaluronic acid–based) can replace lost volume in the cheeks, temples and under-eyes.

  • Biostimulatory injectables can stimulate natural collagen production for natural-appearing, gradual results.

  • Energy-based treatments — including radiofrequency, ultrasound tightening and non-ablative laser resurfacing — improve tone, tighten skin and stimulate deep collagen remodeling.

  • Fat grafting may be appropriate for more advanced cases of facial deflation, using a patient’s own fat to restore volume and create natural, durable results.

“It’s important to intervene early, before dramatic facial changes have taken place,” Dr. Elgash says. “Once the face has lost significant volume, it’s much harder to correct and often requires more aggressive treatment.”
